Your PHI (Protected Health Information) may be used or disclosed to help in your treatment, to get payment for services provided to you and to operate this practice:
What is PHI?
PHI, or Protected Health Information, is information about you, including demographics and even your name. This information may identify you and relate to your health and health care services.
Treatment – We may need to discuss your PHI with others involved in your health care such as other doctors, pharmacy or other family members involved in your health care.
Payment – We may disclose your PHI to collect payment for our services to your insurance company, family members or anyone else responsible for your bill.
Health Care Operations – In order to operate in our business we may have to disclose PHI. For example, calling your name in the waiting room or evaluating the quality of care in a chart review.
Appointment Reminders - We may contact you by mail, telephone, voicemail or email notifying you of an appointment, lab results or office changes.
As Required by Law – We will disclose PHI about you when required by International, Federal, state or local law.
Data Breach Notification Purposes – We may use or disclose your PHI to provide legally required notice of unauthorized access or disclosure of your health information.
Your written Authorization is required for other uses and disclosures for the following:
Most uses and disclosures of psychotherapy notes
Uses and disclosures of PHI for marketing purposes
Disclosures that constitute a sale of your PHI
Your rights regarding your PHI:
Inspect & Copy – You have the right to inspect, receive and copy PHI that may be used to make decisions about your care or payment for your care. We have up to 30 days to make your PHI available to you and we may charge you’re a reasonable fee for the costs of copying, mailing or other supplies associated with your request.
Summary or Explanation – We can provide you with a summary of your PHI rather than the entire record.
Electronic Copy of Electronic Medical Records – If you PHI is maintained in an electronic format, you have the right to request and electronic copy.
Receive Notice of a Breach – You have the right to be notified upon a breach of any of your unsecured PHI.
Accounting of Disclosures – You have the right to ask for an “accounting of disclosures” which is a list of disclosures we made of your PHI. You must submit request in writing.
Paper Copy of this notice – You have the right to a paper copy of this notice even if you have agreed to receive this notice electronically.
